Who qualifies for structured Family Caregiving in Georgia?

If you’re caring for a loved one at home, you may already qualify for family caregiving support. At Harmony Home Private Care, we’re here to make the process simple and stress-free.

WHO IS ELIGIBLE AS A PATIENT

  • The client must be eligible for Medicaid. 

  • The caregiver must live in the same home as the client.

  • Caregivers must be 21 years or older. 

  • Clients can still receive SSDI while participating in the program.

  • They must be enrolled in a CCSP or SOURCE long-term care waiver program.

WHO IS ELIGIBLE AS A CAREGIVER 

  • Caregiver cannot work another job

  • Someone eligible to work in the United States 

  • Must live in Georgia 

  • Spouses are not qualified 

  • Able to direct your own care/services (if not have an individual representative to appoint)

  • Must be at least 18 years old.

  • Must be able to pass a background check
